Our team has advanced training in nursing, geriatrics and mental health as well as grief & loss (hospice) and care management. We use an interdisciplinary whole-person approach to assess issues, tailoring solutions to each individual’s needs while respectfully maintaining the client and family’s integrity and dignity. The team works collaboratively to bring the best possible care to you or your loved one, determining resources and providing support and recommendations for a wide variety of needs and circumstances.

Dianne Reynolds RN, BSN, MA, CMC Professional Care Manager/Founder
As a Professional Care Manager for 23 years, I established FHL Care Management in January of 2010. I have a Bachelor’s Degree in Nursing and a Master’s in Counseling, in addition to holding a Certification as a Professional Care Manager by the National Association of Certified Care Managers. Being a Registered Nurse for over 35 years and having worked in Critical Care, as a Faith Community Nurse in congregations of faith and as a Hospice Chaplain. I have had the privilege of serving people in very difficult times of their lives. I am proud to be a member of the National Aging Life Care Association (ALCA) and the Florida Chapter of the Aging Life Care Association. I am active in the Central Florida Chapter of ALCA. I have found my life calling in working with older adults as it is both challenging and satisfying. There is a special place in my heart in working with clients with dementia. I am married to my awesome husband Roger and between us have three daughters, a son, 4 grandchildren and our golden retriever Abigail. When I am not working, I am cooking, reading and playing with Abby. I also love attending blues concerts with Roger.
Stephanie Smith CDP, CMDCP Managing Director
I have been a member of Team FHL since its founding in 2010. As the Managing Director I oversee daily operations, while implementing our vision and mission by developing strategies and plans to meet goals and objectives for team engagement and company growth. I am a Certified Montessori Dementia Care Professional and have a passion for working with the aging population, especially those with dementia. I am currently pursuing a degree in Social Work, with concentration in Aging Studies. Outside of the office I enjoy spending time with my life partner Zack and our delightful dog Luna. Born and raised in Florida we enjoy going to the springs and anything outdoors in nature. I am a bit of a foodie and enjoy trying new recipes and exploring different cuisines.
Diane Fiore MS, OTR/L Professional Care Manger
I am a Registered Occupational Therapist licensed in the states of Florida and New York, and have a certification from the National Board for Certification in Occupational Therapy. I have been practicing Occupational since 1990 and have extensive experience in sub-acute rehab and home care settings. In addition, I worked as a Quality Management Specialist, and have presented at State and National Conferences on Fall management. I have a Master’s Degree in Health Services Administration which has allowed me to extend my skills as a practitioner in all areas of health care and aging. Currently as a Professional Care Manager, I am able to engage my skills in a variety of ways to help those who need assistance navigating the health care system and managing their own health care issues. I’m married to Brian. He and I spent many years volunteering at YMCA Camp Chingachgook in Lake George, New York at their Y-Knot Sailing program for disabled sailors. We enjoy all time spent outdoors, and love to play golf.
Tracy Sherrod RN, BSN Professional Care Manager
I am a Graduate of the University of Central Florida with a Bachelor of Science Degree in Nursing, I earned my Commission as a Naval Officer in 1990 at Navy Hospital Orlando and served 7 Honorable Years in the United States Navy and 7 inactive years in the Ready Reserve. I am a Registered Nurse of more than 30 years and have worked in many disciplines in nursing from Pediatric, Hematology/Oncology, Recovery Room to Case Management. I am a Professional Care Manager and a Member of the National and local Central Florida Chapter of the Aging Life Care Association. In recent years, I have grown to care for the elderly population dedicated to delivering compassionate and coordinated care, in all stages of their lives. In my spare time, I love spending Saturdays in my backyard garden, cuddling up to a good book or catching up on my sleep. I’m most passionate about the care of my granddaughter, spending time with my family, morning walks with my husband, riding my bike, dancing, listening to a variety of music and volunteering.
Eleanor Hoffarth, MSW Professional Care Manager
I have been working in the senior care industry for 10 years and genuinely enjoy building relationships with my clients and being their advocate. I have a Master’s Degree in Social Work and a Military Social Work Graduate Certificate. Having worked in a variety of senior care facilities (i.e., memory care, skilled nursing care, and independent living) has given me the opportunity to collaborate with community partners and other health care professionals. I am well-informed of the many resources available for my clients in our community and knowledgeable about the physical, mental, and social changes impacting seniors. When I’m not working, I enjoy traveling with my husband, going to Disney World, spending time at the beach, and reading.
Katherine Cummings RN, BSN Professional Care Manager
I have 5 years of experience in the healthcare field, I have a background as a medical assistant and am a registered nurse with specialized experience in the ICU. My experience includes a comprehensive knowledge of the aging population and a deep understanding of the hospitalization process. Originally from Connecticut, I have adapted well to living in Florida for the past 6 years. Outside of work, I enjoy taking long walks around my local park, reading autobiographies and spending quality time with my family and friends.
Carmen Nieves-Deal, ADC Active Enrichment Program Coordinator/Associate Care Manager
I am the Active Enrichment Program Coordinator for FHL Care Management. Prior to taking this role I worked 23 years as Activities Program Manager for Council on Aging in Kissimmee, FL. Adult Daycare Program. Overseeing Therapeutic social activities, medical transportation, hurricane relief, and meals on wheels for adults with functional impairments in a protective environment. Responsible for assisting the Program Director in the overall day-to-day supervision of 40 adults and 15 staff. I began working for Blue Path ABA in Longwood, as a Registered Behavior Tech providing children with autism direct therapy. Helping them with communication, socialization, and daily living skills through treatment plans, play-based therapy, teaching new skills to reduce problem behaviors and reinforce positive behaviors. I am fluent in Spanish and English. I am CPR, Alzheimer’s certified and trained, nationally certified and trained as an Activities Director since 2013.
I was born & raised in New Jersey. Owner/Operator of “Carmen’s Side Street Salon” until she relocated to Florida in 1998. I come from a large family and enjoy family time, cooking, musical theater, reading, traveling, bike riding, gardening, beach, and crocheting. I remarried in 2023 and relocated to Orlando from Saint Cloud, Florida. I have a daughter, Angel, who is 28 years old. I love to serve the Lord and her clients as it has been my lifetime passion driven work.
FHL was recommended to me when I was taking steps to become a guardian as a family member was in need of special assistance from a deep depression. Dianne Reynolds is the owner of FHL and I spoke to her before the guardianship began and she gave me valuable information and assurances that I was doing the right thing and that FHL could help me with many of the things I wasn’t aware of.Once the guardianship was in place, Dianne personally helped me immediately get my family member to a care facility where the recovery process could begin. They slowly improved over the next couple of years, and FHL was there every step of the way to take my loved one to doctor appointments and make suggestions of new care facilities along the way that would meet needs better as improvements began to come.Now this family member is no longer in need of the guardianship, which I realize is a rare thing. Unfortunately I no longer needed the services of FHL as we were fortunate enough to have my family member in completely recovery. FHL had become like family, and it was tough to say our goodbyes to Dianne and her wonderful staff. But FHL was indeed an invaluable asset in my family member’s recovery and I will be forever grateful for that. Thank you Dianne and FHL! Five out of five stars!!!!
