If you care for a person with a medically complex situation, you have many plates spinning at once. You need to coordinate between medical specialists (e.g., a cardiologist, pulmonologist, neurologist, rheumatologist). You often have multiple nonmedical support providers. There may be durable medical equipment to rent or purchase. Possibly lots of medications to manage. Does your loved one have mobility challenges? And what about the always-present worry about needing to be admitted to the hospital?
Our team specializes in stabilizing medically complex individuals. Our goal is to keep them out of the hospital and leading calm, fulfilling lives.
- We provide the oversight, serving as a patient advocate and care coordinator, addressing the medical side of the situation (doctor appointments, prescriptions, and other treatment logistics).
- We are available to do a home safety evaluation and can also offer suggestions for assistive technology, promoting independence and quality of life.
- We can help with emotional concerns and family discord. Also, referrals to available community programs.

Care coordination
Is the person you care for homebound? Perhaps they have mobility challenges. Or are too frail or confused to go to the doctor’s or get blood drawn. They may need advanced medical support, but what they really want is to stay at the place they consider home. Whether they live in a single-family house or an independent-living community, we can arrange for many services to be completed at home (labwork, x-rays, psychotherapy, wound care).
Maybe your relative needs more home-based therapy visits than Medicare will authorize. When the priority is a full recovery, we can arrange for private practitioners to come in as often as is needed.
In addition to service providers, people with medically complex situations often need durable medical equipment (wheelchair, Hoyer lift, hospital bed). Let us help you with the ordering process.
We can also help with reordering common medical supplies. You don’t want to run out! Plus, we use technology for medication management and better adherence.
Looking for home care in the greater Orlando area, Ocala and The Villages® community? We can make recommendations. If you like, we can then serve an oversight function. We work with the provider to be sure the in-home caregivers are providing the care that is needed. Sometimes they just need a little extra training about best practices for dementia or other conditions.
If it seems like long-term care would be the wisest choice, we can also make suggestions for the most appropriate assisted living, memory care, or nursing home.

Nursing homes
Sometimes all that is needed is a short stay with twenty-four-hour access to medical staff to get your family member fully recovered before they go home. At other times, home simply isn’t an option and the wisest choice is a residential nursing home. We assess your loved one’s medical needs and psychosocial health, as well as personal values and priorities, to find a facility that will be the best match. We follow up and monitor our clients to ensure they are receiving high-quality support and will make recommendations for additional care when needed.
